Rates
My current rate is $120 per 50-minute session (First Session is $175. This includes initial evaluation and assessment). Payment (including co-payment) is due at each session by cash or check.
Insurance
I am an in-network provider for Anthem Blue Cross, Aetna, Blue Shield, TriCare (Optum), and Magellan. Services may be covered in part or in full by your health insurance or employee benefit plan. I will bill your insurance for sessions and collect your co-pay at the time of service. To determine if you have mental health care coverage, you should contact your insurance company carrier (usually the customer service number is found on the back of your medical ID card). When checking your coverage, please find out the following information:
- What are my mental health benefits?
- What is the coverage amount per therapy session?
- How much is my co-pay (please bring this with you to each session)
- How many therapy sessions does my plan cover?
- If seeking couples/family counseling is this covered by the plan?
- How much does my insurance pay for an out-of-network provider?
- Is approval required from my primary care physician?
If you have a PPO plan that is not one of the above mentioned insurance companies, you may want to check to see if your plan covers behavioral health services for out of network providers. I provide you with the option to bill the insurance company for your services if you choose.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
